Number of adults and children traveling with you. |

|
Number of beds requested for room. |
|
|
Special dietary considerations. |

When making DINNER PARTY and EVENT reservations, please include the following information: |

Contact telephone number or email address. |
|
|
Where you are staying in the DR. Please include room number if hotel. |

|
Date requested for dinner or event, and alternate date. |
|
|
Approximate times for guest arrival and meals. (Winter sunset at 5:30 p.m.) |

Number of adult and children guests in party. |
|
|
Requests for special menu or dietary considerations. |

|
Any other pertinent information. |
